How To Make a Payment
Please enter your account number followed by the # sign (if this is correct Press 1; if not Press 2)
     1 - To pay total amount due
     2 - To pay a different amount
          For example enter 1 5 0 0 3 # for $150.03 (if this is correct Press 1; if not Press 2)
     3 - To continue (will take you back to payment options menu)
     9 - Repeat this information

Once You Enter Amount You Want To Pay
     1 - Add another account to payment total
     2 - Restart payment process (will take you back to payment options menu)
     3 - Pay amount you have selected

Select Payment Method
     1 - Pay by MasterCard, Visa or Debit Card
     3 - Exit this menu with making a payment (will take you back to payment options menu)
     9 - Repeat these options

Credit or Debit Card as your Payment Method
     1 - If correct, Enter credit card number; Enter expiration date; Enter security code on back of card and Press #
     2 - Review your payments
     3 - Restart payment process (will take you back to payment options menu)
     9 - Repeat these options

Account Status Menu

Create or Update PIN Number
Please enter your account number followed by the # sign (if this is correct Press 1; if not Press 2)
     1 - Enter the last 4 digits of your social security number to verify identity
     2 - Enter desired four digit PIN number

Customer-Owned Pay Telephone Service

• Per Access Line, each: $22.22

o Originating Line Screening: $5.00

o Billed Number Screening: No Charge

o Coin Supervision Additive: $2.21

Touch Tone rate:

• Residential: $2.00

• Business: $2.00

Other Services Rates

Wire Maintenance rate:

• Residential (per access line): $1.50

Service Ordering

Service Order Charge

o Primary Service Order Charge - A charge for connecting new or additional central office lines, per service order.

• Residential Primary Service Order Charge - $10.00

• Business Primary Service Order Charge - $10.00

o Secondary Service Order Charge - A charge for moving or changing existing service and equipment or adding new or additional service and equipment other than central office lines, per service order.

• Residential Secondary Service Order Charge - $10.00

• Business Secondary Service Order Charge - $10.00

Access Line Work

o Central Office Connection Charge – A Charge for required connection and testing in the central office for line, numbers, connectors, and main distribution or the equivalent.

• Residential Central Office Connection Charge - $11.00

• Business Central Office Connection Charge - $14.00

Maintenance of Service Charge

o Maintenance of Service Charge – A charge for each visit by Company personnel to the customer’s premises where difficulty or a trouble report results from malicious destruction of Company-owned terminal equipment or wiring and the unauthorized tampering with Company-owned terminal equipment or wiring and trouble caused by the customer owned equipment or wiring.

• Residential Maintenance of Service Charge - $25.00

• Business Maintenance of Service Charge - $25.00

Premise Visit Charge

o Premise Visit Charge – A charge applied when a Company employee makes a field trip (including but not limited to the customer’s premises) to perform requested work by the customer, other than disconnect work.

• Residential Visit Charge - $25.00 initial charge plus $15.00 for each quarter hour.

• Business Visit Charge - $25.00 initial charge plus $15.00 for each quarter hour.

Restoral of Service Charge

• Restoral of Service Charge – applies for the re-establishment of service for a subscriber whose service has been discontinued by the Company for non-payment of bills. Service is restored as it was prior to disconnection. If the subscriber desires any changed in Service, the service will be billed accordingly. The Restoral of service charge is paid in advance, and any difference in service upon restoral will be billed at applicable charges.

o Residential Restoral of Service Charge - $15.00

o Business Restoral of Service Charge - $15.00

Returned Check Charge

• Returned Check Charge – applies for each returned check or other forms of payment where moneys are not honored by a bank or depository.

o Residential Returned Check Charge - $25.00

o Business Returned Check Charge - $25.00
